Posterior surface anatomy of the uterus is rendered with emphasis on the facies intestinalis (intestinal surface) of the uterine body and cervix. Superiorly, the uterine fundus transitions into the posterior wall of the corpus uteri, while inferiorly the cervix forms a more cylindrical segment that projects toward the vaginal fornix. Immediately posterior to the cervix, the peritoneum reflects onto the anterior rectum to create the rectouterine pouch (pouch of Douglas), with its lowest point indicated just behind the supravaginal cervix in the midline. That peritoneal recess matters in day-to-day gynecology. The rectouterine pouch is the most dependent portion of the female peritoneal cavity in the supine pelvis, so blood, pus, or ascites often collects here and can be targeted by transvaginal ultrasound assessment or culdocentesis via the posterior vaginal fornix. It is also the anatomic space distorted by endometriosis and adhesions, and it becomes a key landmark during posterior colpotomy, total laparoscopic hysterectomy, and dissection of the rectovaginal septum when separating cervix from rectum. Use this posterior view in pelvic anatomy teaching for medical and midwifery curricula, in OB-GYN operative atlases illustrating entry into the pouch of Douglas, and in radiology or ultrasound training materials that correlate peritoneal reflections with free fluid in the posterior cul-de-sac.
