Posterior uterine surface (facies intestinalis uteri) is rendered in 3D, with the fundus and superior body lying cranial to the lower uterine segment and cervix. The perimetrium forms a smooth convex contour that faces the rectouterine pouch, while the sacrouterine (uterosacral) folds begin near the posterolateral cervix and sweep posteriorly toward the sacrum. Inferiorly, the cervix projects into the upper vagina, and laterally the broad ligament attachment margins define the transition from the free posterior wall to the ligamentous reflections. Clear midline symmetry anchors orientation. That posterior face matters because it is the operative and imaging context for cul-de-sac disease. Endometriotic implants and adhesions often collect along the uterosacral ligaments and posterior cervix, tethering the uterus and narrowing the pouch of Douglas, findings correlated with deep dyspareunia and difficult uterine mobilization at laparoscopy. During total hysterectomy and endometriosis excision, the uterosacral complex guides dissection near the ureter as it courses anteromedial in the pararectal space. Small landmarks. Big consequences. Use this asset in gynecologic anatomy teaching to contrast the facies intestinalis with the anterior vesical surface and to introduce peritoneal reflections, rectouterine pouch boundaries, and uterosacral ligament topography.
