Description
Serosal reflection over the adult female urinary bladder is rendered as a continuous visceral peritoneal membrane draping the superior surface (apex through body) and curving posteriorly toward the pelvic peritoneum. The peritoneum stops short of the inferior bladder, leaving the base (fundus) and neck predominantly extraperitoneal as they approach the urethra and anterior vaginal wall. Anteriorly, the bladder lies deep to the pubic symphysis in the retropubic space, while posterior relations sit closer to the uterus and upper vagina, where the peritoneum forms a dependent recess between organs. Peritoneal coverage matters because it defines where bladder distension can elevate the peritoneal dome and where surgical entry risks a peritoneal breach. A transabdominal suprapubic approach to cystotomy or trocar placement must respect the superior peritoneal reflection, and a low, empty bladder increases the chance of inadvertently opening peritoneum and entering the peritoneal cavity.
