Description
Anteriorly in the adult female pelvis, the urinary bladder is presented with emphasis on its serous coat (tunica serosa), the peritoneal investment that covers the superior surface and extends onto the posterior aspect near the bladder dome (apex) while leaving much of the anterior and inferior bladder extraperitoneal. Reflections of the parietal peritoneum onto the visceral peritoneum define the vesicouterine pouch, positioning the serosa posterior and superior to the bladder body and anterior to the uterus. Around the margins where peritoneum ends, the serosal layer transitions abruptly to adventitia, a boundary that helps orient the fundus (base) and the relationships toward the pubic symphysis anteriorly and pelvic floor inferiorly. Serosa is limited. That limitation matters. Peritoneal coverage of the female bladder is a practical landmark for both anatomy teaching and operative planning. During suprapubic cystostomy or open retropubic access, surgeons aim to enter the bladder through the extraperitoneal anterior surface to avoid breaching the peritoneal cavity, whereas dissection near the bladder dome risks peritoneal entry and potential urinary leakage into the peritoneal space. The same serosal reflections are also the reason intraperitoneal bladder rupture classically involves the dome, while extraperitoneal rupture more often tracks around the bladder neck and anterior wall after pelvic trauma.
