Description
Gross anatomy centers on the adult female urinary bladder with emphasis on its serous membrane (tunica serosa) where peritoneum reflects onto the superior surface and dome. The peritoneal covering occupies the superoposterior aspect, while the anterior surface, inferolateral surfaces, and bladder neck remain extraperitoneal and blend into adventitia against the retropubic space. Superiorly, the serosa relates to the peritoneal cavity, and posteriorly it approaches the vesicouterine pouch between bladder and uterus, a key reflection point in the female pelvis. Serosal margins are typically sharp enough to contrast with the underlying detrusor contour and the transition toward the trigone inferiorly. Mapping the complete extent of bladder serosa matters when you plan or interpret pelvic surgery, because peritoneal entry and urine leak risk depend on whether an incision crosses a peritoneal reflection. During abdominal hysterectomy, cesarean delivery, or vesicovaginal fistula repair, the vesicouterine peritoneal fold is dissected to mobilize the bladder inferiorly, and an accurate mental model of where serosa ends helps avoid inadvertent cystotomy or peritonitis. Overdistension, pelvic adhesions, and prior surgery can pull the peritoneal reflection caudally, changing the apparent “safe” extraperitoneal zone. A small border with big consequences.
