Description
Anteriorly in the female pelvis, the urinary bladder is presented from the outside, emphasizing its serosal (peritoneal) covering over the superior surface and dome, with the more inferior bladder base transitioning to adventitia where it contacts pelvic fascia. The peritoneum reflects posteriorly from the bladder onto the uterus, creating the vesicouterine pouch, and laterally the serosa blends toward the pelvic sidewalls near the broad ligament attachments. Inferiorly, the bladder neck aligns with the proximal urethra, while the apex points superiorly toward the anterior abdominal wall where the median umbilical ligament (urachus remnant) typically courses. Surface anatomy. Clean planes. Understanding where true serosa exists on the bladder matters when you are predicting how disease spreads and how safely you can dissect. Peritoneal involvement in urothelial carcinoma changes staging and surgical planning, and inflammatory or iatrogenic perforations at the bladder dome tend to communicate with the peritoneal cavity, unlike extraperitoneal injuries around the base and anterior wall that track into the retropubic space. This superficial view also supports teaching the peritoneal reflections that define the vesicouterine pouch, a common landmark in pelvic surgery and gynecologic imaging.
