Description
Anterior pelvic anatomy centers on the adult female urinary bladder, with its serosal (peritoneal) coat covering the superior surface and reflecting posteriorly toward the uterine region, while the anterior and inferior bladder surfaces remain nonperitoneal where the organ is invested by adventitia and endopelvic fascia. The dome (apex) lies anterosuperiorly, the base (fundus) faces posteroinferiorly, and the serosal reflection marks the boundary between intraperitoneal peritoneum and extraperitoneal connective tissue along the bladder body. Laterally, the peritoneum drapes toward the pelvic sidewall, framing the bladder in relation to the paravesical spaces. Attention to the bladder serosa matters because it defines surgical planes and predicts routes of disease spread. During abdominal or laparoscopic entry into the pelvis, recognizing where peritoneum reflects off the bladder reduces the risk of cystotomy when developing the vesicouterine pouch and mobilizing the bladder for hysterectomy or cesarean delivery. Tumor staging also leans on this anatomy: invasion beyond muscularis into perivesical fat differs from serosal involvement, and peritoneal breach changes patterns of peritoneal seeding and postoperative complications.
