Description
Seen from the left side, the maternal torso is rendered translucent so the gravid uterus sits clearly within the pelvis and lower abdomen. The fetus at approximately 15 weeks of gestation lies in a flexed attitude inside the amniotic sac, head directed anteroinferiorly, with the umbilical cord looping from the abdominal wall toward the placental disc applied to the uterine wall. The uterine fundus now rises above the pelvic brim, displacing coils of small intestine superiorly and posteriorly. Behind and below, the urinary bladder lies anterior to the uterus and the rectum posterior to it, with the vagina continuing inferiorly from the cervix and the bony pelvis indicated as a faint outline. At this stage of the second trimester the fetus has recognisable facial features, limbs with formed digits, and active swallowing and skeletal ossification, while the uterus becomes palpable between the symphysis pubis and the umbilicus. The lateral projection is the most direct way to demonstrate why fundal height measurement works and how the enlarging uterus begins to compress the bladder and alter bowel position, both common sources of maternal urinary frequency and constipation. It also frames the anatomy relevant to second-trimester ultrasound assessment, quadruple serum screening, and amniocentesis, where placental site and amniotic fluid volume determine the approach.
