Description
The maternal torso is shown in left lateral profile, the skin and abdominal wall rendered semi-transparent so that the gravid uterus and its contents read clearly within the pelvis. A fetus at about 16 weeks of gestation lies curled within the amniotic sac, head flexed and limbs drawn toward the trunk, with the umbilical cord looping from the abdomen toward the placenta implanted on the uterine wall. Behind and below the uterus the rectum descends to the anal canal, while the urinary bladder sits anteriorly behind the pubic symphysis and the vagina passes inferiorly from the cervix. Loops of small intestine and the colon are displaced superiorly and posteriorly by the enlarging fundus, which at this stage has risen out of the true pelvis toward the level midway between symphysis and umbilicus. At 16 weeks the second trimester is well established: the fetus is fully formed in outline, movements begin though they are rarely felt, and the uterine fundal height becomes a usable clinical measure of growth. This lateral projection illustrates the mechanical consequences of uterine enlargement, including anterior bladder compression with reduced functional capacity and urinary frequency, upward displacement of bowel loops, and the shift in maternal centre of gravity that contributes to lumbar lordosis and low back discomfort. The relationship of the uterus to bladder, rectum and pubic symphysis also underpins interpretation of second-trimester ultrasound and the timing of screening such as maternal serum testing and detailed anatomy scans.
