Description
A full-term fetus at approximately 39 weeks of gestation lies in a flexed, head-down attitude within the intact amniotic sac, viewed from the left side through the translucent chorioamniotic membranes. The vertex is directed inferiorly and to the left of the frame, the scalp carries fine dark hair, and one hand is raised to the face while the thighs and legs are drawn tightly against the abdomen, with the plantar surfaces and toes of one foot visible. Subcutaneous fat has smoothed the body contours typical of term maturity. The umbilical cord passes from the abdominal insertion at the navel toward the discoid placenta at the right, whose fetal surface and chorionic vessels sit against the vascularised uterine wall, and particulate vernix is suspended in the surrounding amniotic fluid. At 39 weeks the pregnancy is at early term, and this depiction supports discussion of fetal lie, presentation and attitude, the parameters assessed by Leopold manoeuvres and confirmed on ultrasound before labour. The cephalic presentation with a well-flexed head shown here is the configuration that permits optimal engagement of the smallest suboccipitobregmatic diameter in the maternal pelvis.
