Description

At approximately 38 weeks of gestation the fetus fills the uterine cavity, shown here in a flexed attitude with the thighs drawn onto the abdomen, one foot rising toward the chest and the hands gathered near the face. The head is large in proportion to the trunk and covered with dense scalp hair; the external ear, eyelids, nail-bearing fingers and toes and the plump subcutaneous fat of the limbs are all mature features of a term infant. The umbilical cord loops across the torso, its two coiled umbilical arteries and single umbilical vein visible through the amnion-covered Wharton's jelly, running to the central insertion of the discoid placenta. The placenta is displayed in profile against the uterine wall, its fetal surface showing radiating chorionic vessels, while fine endometrial vasculature grades into the surrounding myometrium. At term the anatomy in this view underpins routine obstetric assessment: fetal lie and presentation, placental site and cord position are the same relationships evaluated on ultrasound before labour. Loose nuchal or body cord loops are common and usually benign, but their depiction supports discussion of cord entanglement, true knots and cord compression during descent. The placental profile is equally relevant to placenta praevia, abruption and adherent placentation, and to management of the third stage of labour. The generous subcutaneous fat and mature skin also illustrate why term infants thermoregulate and feed more effectively than preterm neonates. The image suits obstetric and midwifery teaching on term pregnancy, fetal presentation and delivery planning, and complements earlier members of this series showing the second-trimester fetus so that growth across gestation can be compared side by side.
