Lateral orientation highlights the vermiform appendix arising from the posteromedial wall of the caecum, just inferior to the ileocaecal junction where the terminal ileum meets the large bowel. From this side view, the appendix projects as a narrow, blind-ending tube whose base sits at the convergence of the taeniae coli, while its tip commonly angles inferiorly toward the pelvis or posteriorly behind the caecum. A short mesoappendix typically fans from the ileal mesentery to the appendix, carrying the appendicular artery along the free margin. Spatially, the caecum lies anterior and inferior in the right iliac fossa, with the appendix variable in position but consistently tethered proximally to the caecal base. Appendiceal position is not academic trivia, it changes the exam, the differential, and the operative plan. A retrocaecal appendix can produce flank or back-dominant pain and may irritate the psoas, while a pelvic appendix more often refers pain to the suprapubic region and can mimic urinary or rectal pathology.
