Description
Shown in lateral profile, the terminal large intestine is traced from the sigmoid colon into the rectum and anal canal, with the cecum positioned in the right lower quadrant and the vermiform appendix arising from its posteromedial wall just inferior to the ileocecal junction. The appendix projects as a blind-ended, worm-like tube, its tip oriented variably but typically directed inferiorly toward the pelvis in the adult male. Posteriorly, the rectum follows the sacral concavity before turning inferiorly to the anorectal junction. A lateral side presentation helps clarify why appendiceal pain can behave unpredictably: the appendix can lie retrocecal and posterior, or descend into the pelvis where it may irritate the bladder or pelvic peritoneum and shift tenderness away from a classic McBurney point pattern. Orientation matters during appendectomy, where the taeniae coli on the cecum converge at the appendiceal base, a dependable landmark when the tip is hidden by the cecum or peritoneal folds. This is the anatomy behind atypical appendicitis.
