Description
Seen in left lateral profile, the left border (margo sinister) of the heart forms a smooth, convex contour created chiefly by the left ventricle, tapering superiorly toward the left auricle of the left atrium. Superior and slightly posterior to this ventricular prominence, the pulmonary trunk and ascending aorta occupy the superior mediastinum, while the left lung and its mediastinal pleura lie lateral to the pericardial sac. Inferiorly, the left ventricular apex projects anteroinferiorly toward the left hemidiaphragm. A clean silhouette. Clinically, this border is the one that dominates the left cardiac contour on a frontal chest radiograph and shifts predictably with left ventricular dilation, aneurysm, or cardiomyopathy. Correlating the lateral surface anatomy with imaging helps when you are assessing cardiomegaly versus pericardial effusion, since effusions tend to enlarge the overall cardiac silhouette without reproducing the focused bulge of an enlarged left ventricle. It also anchors the teaching point that the apex beat in the left fifth intercostal space at the midclavicular line reflects left ventricular position, not atrial anatomy.
