Description
Viewed from the left lateral aspect, the heart’s left border forms an obtusely rounded contour dominated by the left ventricle, with the left atrial appendage contributing superiorly near the base. The apex lies inferior and anterior, projecting leftward as the left ventricle tapers distally. Posterior to this margin, the left ventricle curves toward the diaphragmatic surface, while the superior portion approaches the atrioventricular groove where the left atrium sits more posteriorly. Stripping away the great vessels focuses attention on the true cardiac silhouette, the same margin that creates the left cardiac contour on a frontal chest radiograph and is assessed in cardiomegaly. Subtle changes in this border can reflect left ventricular hypertrophy from long-standing systemic hypertension, dilated cardiomyopathy, or remodeling after myocardial infarction, and the absence of vascular structures prevents confusion between myocardial contour and the adjacent pulmonary trunk or aortic knob. Clean margins matter.
