Description
Arising from the lateral uterine wall, the uterine venous plexus collects blood from the myometrium and endometrium and converges into paired uterine veins that course within the base of the broad ligament (parametrium). Along the uterine cornu, small venous channels communicate with the ovarian venous plexus near the uterine tube and ovary, while inferiorly the plexus extends toward the cervix and upper vagina. The uterine veins typically run with the uterine artery and pass medially across the pelvic sidewall before draining to the internal iliac venous system. Dense, valveless-leaning communications dominate. A network, not a single trunk. Understanding the outer configuration of uterine venous pathways matters in pelvic surgery because these thin-walled veins are the usual source of brisk bleeding during hysterectomy, myomectomy, and cesarean hysterectomy for placenta accreta spectrum. Their close relationship to the ureter in the parametrial tunnel, and their continuity with vaginal and ovarian venous plexuses, explains why venous injury can be diffuse and difficult to control with a single ligature.
