Description
Multiple uterine veins course along the lateral aspect of the uterus within the broad ligament, draining the myometrium and endometrium as they converge toward the uterine venous plexus at the level of the cervix and lower uterine segment. Tortuous, wide-caliber channels run parallel and inferior to the uterine artery near the uterine cornu and along the uterine body, then track laterally toward the pelvic sidewall. Communicating tributaries link the uterine plexus with the vaginal venous plexus inferiorly and with the ovarian venous network superiorly near the adnexa, forming a continuous pelvic venous bed. Orientation matters. Venous anatomy here becomes clinically unforgiving because the uterine veins are thin-walled, valveless in many segments, and distend markedly in pregnancy, so bleeding during hysterectomy or cesarean delivery often comes from a torn venous plexus rather than an arterial branch. The relationship to the ureter is the key teaching point: the ureter passes inferior to the uterine artery (water under the bridge) and then lies close to the uterine veins at the base of the broad ligament, where clamping and ligation can injure either structure. Pelvic congestion syndrome and postpartum hemorrhage discussions also benefit from a clear depiction of how uterine drainage communicates with adjacent plexuses.
