Description
Along the lateral uterine wall, the uterine venous plexus forms a dense network within the broad ligament, with tributaries coursing in close company to the uterine artery and its cervicovaginal branches. Superiorly, veins from the uterine fundus and cornual region communicate with the utero-ovarian pathway toward the ovarian veins, while inferior channels drain the cervix and upper vagina into the uterine and vaginal venous plexuses. Medial vessels hug the myometrium and cervix, and more lateral trunks converge toward the internal iliac vein via the uterine vein. Small cross-connections span the parametrium and periureteral tissues. Complex, but orderly. Venous anatomy here matters because uterine venous channels are thin-walled, valveless, and prone to brisk bleeding when the parametrium is mobilized. During hysterectomy or uterine artery ligation, the ureter passes inferior to the uterine artery near the cervix (water under the bridge), and the adjacent uterine veins and plexus often obscure that landmark, raising the risk of ureteral injury and uncontrolled venous hemorrhage. Pregnancy and the postpartum state further distend these veins, and dilated uterine and ovarian venous pathways contribute to pelvic congestion syndrome and the collateral patterns seen with iliac or ovarian vein reflux.
