Description
Venous drainage blankets the adult uterus as a dense uterine venous plexus spread over the fundus and corpus, with broad interconnections that track along the lateral uterine walls. Prominent collecting channels converge inferiorly toward the cervix and superiorly toward the uterine cornua, where venous pathways communicate with the tubal and ovarian venous networks. Laterally, the largest trunks run within the parametrium adjacent to the uterine artery at the level of the cervix, then continue toward the internal iliac venous system via the uterine veins. Anterior relations place this plexus deep to the vesicouterine reflection, while posteriorly it lies toward the rectouterine region. Surface mapping of uterine veins matters because venous bleeding is often the limiting factor during pelvic surgery rather than arterial inflow, and the plexiform pattern makes point control unreliable when planes are violated. During hysterectomy, uterine artery ligation occurs near the cervix where the ureter passes inferior to the artery, but the accompanying uterine veins can be thin-walled, variable, and prone to avulsion in traction, a common mechanism for brisk pelvic hemorrhage. The same venous pathways contribute to pelvic congestion syndrome and can provide a route for septic pelvic thrombophlebitis postpartum.
