Description
Superficial uterine venous channels course across the serosal surface of the uterine body and fundus, converging toward the lateral uterine margins where they communicate with the broad ligament plexus. Smaller veins descend along the uterine isthmus and cervix, tracking inferiorly toward the vaginal venous plexus, while more superior tributaries approach the adnexal region and anastomose with ovarian venous pathways. Medially, the network drains the myometrium and endometrium through penetrating venules that pass outward to the subserosal plexus. Paired patterns mirror the arterial distribution, with venous routes running parallel to the uterine artery and its arcuate and radial branches as they traverse the myometrium. Venous anatomy is the teaching hinge for postpartum hemorrhage and pelvic congestion, because the uterus can bleed briskly from low pressure but high capacity channels that are difficult to tamponade when they retract into myometrium. During hysterectomy, the uterine veins at the level of the cervix lie in close proximity to the ureter as it passes inferior to the uterine artery near the cardinal (transverse cervical) ligament. That relationship drives clamp placement and explains why unrecognized venous tributaries can obscure the operative field. Expect congestion and dilatation in pregnancy.
