Description
Seen from the posterior aspect of the heart, the left atrium forms a broad dome receiving the pulmonary veins, with paired right and left venous orifices entering the atrial wall superior to the mitral vestibule. The right pulmonary vein openings sit to the patient’s right of the interatrial septal region, while the left pulmonary vein openings occupy the left posterolateral atrial wall. Each opening leads into the pulmonary vein lumen, and their ostia cluster high on the atrial dome relative to the atrioventricular junction. Orientation is everything. Posterior left atrial anatomy matters because the pulmonary vein ostia anchor most contemporary atrial fibrillation strategies, from circumferential pulmonary vein isolation to posterior wall debulking and roof line lesions. Small differences in the positional arrangement of these openings, including a common left pulmonary venous trunk or closely spaced ipsilateral ostia, change catheter stability, lesion set planning, and the risk profile for pulmonary vein stenosis. The posterior relationship to the esophagus is also clinically relevant when delivering radiofrequency energy along the posterior left atrial wall.
