Presented in sagittal section, the vagina is shown investing the cervix of the uterus so that the superior vaginal lumen forms fornices around the portio vaginalis. The posterior vaginal fornix lies inferior to the posterior cervical lip and anterior to the rectouterine pouch (pouch of Douglas), creating the deepest recess; the anterior fornix sits between the anterior cervical lip and the vesicouterine reflection, and the lateral fornices flank the cervix on either side. Superior to the fornices, the cervical canal continues into the uterine cavity, while inferiorly the vaginal canal extends toward the vestibule. This relationship matters because the posterior fornix is the most accessible peritoneal recess from the vagina, a fact that underpins culdocentesis and transvaginal drainage of pelvic abscess in the rectouterine pouch. Depth and contour of the fornices also change with uterine position (anteversion versus retroversion) and cervical enlargement, and that alters what you can safely palpate on bimanual exam or reach during posterior colpotomy.
