Description

Posteriorly in the mid-sagittal section, the vaginal canal ascends to the cervix uteri, where the vaginal part of the cervix (portio vaginalis) projects into the lumen and creates the vaginal fornix. The anterior fornix lies between the cervix and the posterior surface of the urinary bladder, while the posterior fornix extends superiorly and posteriorly toward the rectouterine pouch (pouch of Douglas) across the posterior vaginal wall. Superior to the fornices sits the uterine cervix continuing into the uterine body, with the vaginal axis angled posteriorly relative to the uterine axis. Relationship is the point. This sagittal perspective matters because the posterior fornix is the closest vaginal route to the peritoneal cavity, separated only by the rectovaginal septum and peritoneum of the rectouterine pouch, which is why posterior colpotomy and culdocentesis target this recess. Small shifts in uterine version and flexion change the depth and accessibility of the fornices, a practical teaching point when correlating bimanual pelvic examination with speculum positioning.
