Description
Rendered in an x-ray style transparency, the male heart is presented with the right and left atria positioned superior to their corresponding ventricles and the great vessels emerging from the cardiac base. The sinoatrial (SA) node sits high in the right atrium near the superior vena cava, while the atrioventricular (AV) node lies more inferiorly in the interatrial septum at the apex of the triangle of Koch, just superior to the septal leaflet of the tricuspid valve. From the AV node, the His bundle penetrates the fibrous skeleton into the interventricular septum, where right and left bundle branches descend toward the apex and arborize as Purkinje fibers along the subendocardium of both ventricles. Clear spatial cues. This configuration matters because the AV node and penetrating His bundle mark the only physiologic electrical bridge across the insulating annulus fibrosus, a fact that explains both the PR interval on ECG and common mechanisms of AV block. The relationship of the AV node to the membranous septum and tricuspid annulus is the surgical hazard zone during aortic or tricuspid valve procedures, and it is also the anatomic substrate targeted in catheter ablation for AV nodal re-entrant tachycardia (slow pathway region near the coronary sinus ostium). Bundle branch distribution also frames how septal infarction can produce new right bundle branch block or left anterior fascicular block.
