Description

Rendered in an x-ray style with semi-transparent myocardium, the male human heart is presented in an oblique three-quarter view so the atria and ventricles can be read through the valve plane and interventricular septum. The sinoatrial node sits at the superior posterolateral right atrium near the junction of the superior vena cava, while the atrioventricular node lies inferiorly within the interatrial septum near the coronary sinus ostium and septal leaflet of the tricuspid valve. From there, the atrioventricular bundle (bundle of His) penetrates the fibrous skeleton, courses along the membranous septum, and divides into right and left bundle branches that descend subendocardially toward the ventricular apices, with Purkinje fibers spreading distally across the ventricular walls. Autonomic pathways are overlaid as color-coded nerve networks tracking to the cardiac plexus and along the great vessels (ascending aorta, pulmonary trunk, pulmonary veins). This angle gives a clean read on the relationship between the AV node, the membranous septum, and the proximal conduction tissue, a common site for iatrogenic injury during aortic valve replacement, TAVR, and closure of perimembranous ventricular septal defects. The spatial separation of the SA node from the AV junction also helps explain atrial arrhythmia mechanisms and why right atrial incisions, cannulation, or SVC isolation can precipitate sinus node dysfunction.
