Description

Arising from the venous plexuses of the dura mater, the middle meningeal vein courses in the epidural space along the inner table of the calvaria, closely accompanying the middle meningeal artery (MMA) after it enters the cranial cavity via the foramen spinosum. Branches spread superiorly and posteriorly over the lateral cranial vault, running in parallel with the arterial grooves and lying deep to the squamous part of the temporal bone and adjacent sphenoid. Superficial cerebral gyri and sulci are rendered for orientation while cranial and upper cervical nerves radiate inferiorly from the brainstem and spinal cord. Relationships stay clinically intuitive: dura and meningeal vessels internal to bone, neural elements more centrally placed. Course matters. Meningeal vessels tethered to the skull are vulnerable in lateral head trauma, and while epidural hematoma is classically arterial from the MMA, injury to the middle meningeal vein or its dural tributaries can contribute to epidural bleeding and complicate imaging interpretation when the clinical story and CT density do not fit the textbook pattern. The vein’s proximity to the pterion region and its entry at foramen spinosum also makes it a realistic structure to anticipate during extradural approaches and when drilling the sphenoid ridge.
