Description
Arcing laterally across the dorsum of the foot, the arcuate artery is shown arising from the dorsalis pedis artery just distal to the ankle and running over the bases of the metatarsals. From its convex border, dorsal metatarsal arteries branch distally toward the toes, where they continue as dorsal digital arteries along the margins of the digits. Proximal perforating branches are typically suggested by their course toward the plantar arterial arch, linking dorsal and plantar circulations. Superficial dorsal veins accompany the arterial pattern more variably, often converging medially toward the great saphenous vein and laterally toward the small saphenous system. Clean relationships. This dorsal vascular arrangement matters when you are localizing the dorsalis pedis pulse and assessing distal perfusion in peripheral arterial disease, diabetes, or after trauma to the midfoot. The arcuate artery and its dorsal metatarsal branches sit at risk in dorsal approaches for Lisfranc injury fixation and midfoot arthrodesis, where incision placement and retractor positioning can compromise small-caliber vessels and contribute to wound-healing problems. It also helps explain why embolic or atherosclerotic disease may spare one toe while affecting another, depending on branching and collateral flow via perforators to the plantar arch.
