Description
Seen from an anterior (dorsal) angle, the forefoot and midfoot are organized around the tarsus, metatarsus, and phalanges, with the extensor tendons crossing the ankle and continuing distally over the metatarsal shafts toward the toes. Muscle bellies and tendinous slips layer across the dorsum, while the tarsal and metatarsophalangeal ligamentous restraints appear as pale bands spanning adjacent bones and joint capsules. Medially, structures align toward the hallux; laterally, they taper toward the fifth ray. Bony landmarks anchor the soft tissue relationships. Dorsal foot anatomy matters because it is where long extensor tendons, retinacular bands, and neurovascular structures share a tight corridor that clinicians palpate, inject, and incise. Painful tenosynovitis of extensor digitorum longus or extensor hallucis longus, dorsal midfoot osteophytes causing shoe pressure, and compartment syndrome of the dorsal interossei all require you to orient quickly to tendon paths relative to the metatarsals and tarsometatarsal joints. This angle also supports teaching of toe extension mechanics, including how tendon slips and intrinsic muscles contribute to metatarsophalangeal extension and interphalangeal extension.
