Description
Arising from the lateral circumflex femoral artery, the descending branch is traced from a medial viewing direction as it passes deep to the sartorius and rectus femoris, then courses distally along the anterolateral thigh on or within the substance of vastus lateralis. Proximal relationships to the femoral artery and profunda femoris artery are suggested at the level of the femoral triangle, with paired venae comitantes running in close apposition to the artery. Distally, the vessel approaches the lateral aspect of the knee, where genicular branches contribute to the periarticular network and potential genicular anastomosis. For operative planning, this artery matters because it anchors the vascular pedicle for the anterolateral thigh (ALT) flap, and its perforators to the skin and fascia can be septocutaneous or musculocutaneous as they traverse the vastus lateralis. Surgeons routinely use a medial-to-lateral mental map when dissecting in the intermuscular planes and when avoiding iatrogenic injury during approaches to the distal femur or lateral retinacular procedures around the knee. Collateral flow through the descending branch and genicular channels also becomes clinically relevant when superficial femoral artery disease shifts perfusion demands toward profunda femoris derived pathways.
