Description

Arising as a terminal branch of the subscapular artery, the thoracodorsal artery descends along the posterior axillary wall and tracks inferiorly on the deep surface of latissimus dorsi. From a posterior perspective, it lies medial to the surgical neck region of the humerus, closely related to the lateral border of the scapula and the posterior ribs that form the thoracic cage. Distally, the vessel resolves into multiple arteriolar branches that fan out within the muscle belly toward the inferolateral back. Small-caliber ramifications are the point. For reconstructive surgeons, this course matters because the thoracodorsal artery forms the dominant pedicle for the latissimus dorsi flap, with its caliber and length supporting pedicled transfer to the anterior chest or free tissue transfer. Injury or ligation can occur during axillary lymph node dissection or posterior axillary approaches, and awareness of its descent helps prevent ischemia of the latissimus dorsi and reduces bleeding along the posterior chest wall.
