Description
Prominent in this small intestine wall cutaway are the mucosa thrown into circular folds (plicae circulares) and covered by finger-like intestinal villi projecting into the lumen. A simple columnar epithelium with interspersed goblet cells lines each villus, while the lamina propria core carries capillary loops and a central lacteal that drain toward the submucosa. Deep to the muscularis mucosae, the submucosa separates the mucosa from the muscularis externa, where an inner circular and outer longitudinal smooth muscle layer encircle the bowel, with enteric nerve plexuses and accompanying arteries and veins coursing between layers in expected anatomic planes. That layered arrangement matters because many diseases track along these interfaces: villous blunting and crypt hyperplasia in celiac disease flatten the absorptive surface, while ischemia first injures the metabolically active mucosa supplied by terminal arterioles in the lamina propria. Surgeons and endoscopists also think in these strata when interpreting ulcer depth, bleeding source, or the risk of perforation, since lesions confined to mucosa and submucosa behave differently from transmural Crohn disease that extends through muscularis externa and serosa.
