Description
Centered in the upper abdomen, the pancreas is rendered posteriorly as a retroperitoneal gland extending transversely from the duodenal C loop on the right to the splenic hilum on the left. The head sits inferior to the liver and gallbladder and medial to the second portion of the duodenum, while the body crosses anterior to the great vessels and the tail approaches the spleen. A focal neoplastic mass is shown deep to the stomach, near the expected course of the main pancreatic duct, with close proximity to the distal common bile duct as it enters the duodenum. Color differentiation keeps the liver, stomach, small intestine, and large intestine readable as surrounding landmarks. Posterior anatomy is where pancreatic cancer declares its resectability. Tumors of the head commonly compress the intrapancreatic bile duct, producing painless obstructive jaundice, and they can encase the superior mesenteric artery or invade the portal vein and superior mesenteric vein confluence, shifting management from a Whipple procedure to neoadjuvant therapy or palliation. Direct retroperitoneal extension also explains early back pain and difficulty achieving negative margins, since the posterior pancreatic surface abuts fascia, lymphatics, and perivascular nerve plexuses. Endocrine consequences matter too, because loss of islet tissue can worsen glucose intolerance and insulin dependence.
