Description
Arising from the pulmonary trunk, the right pulmonary artery courses horizontally to the patient’s right, passing posterior to the ascending aorta and superior to the left atrium as it approaches the right lung hilum. At the root of the right lung it lies anterior to the right main bronchus, then divides into lobar and segmental branches that accompany the bronchial tree into the lung parenchyma. The trachea and main bronchi provide superior, midline airway landmarks, while the cardiac silhouette and coronary vessels sit inferior and medial to the proximal pulmonary arterial segment. Color coding distinguishes the pulmonary arterial pathway from adjacent systemic arteries and the pulmonary veins returning to the left atrium. This relationship matters in day to day thoracic imaging and in the operating room. On CT pulmonary angiography, the right pulmonary artery is a common site for embolus lodgment and a frequent reference point when tracing clot burden into lobar and segmental branches. Its position relative to the right main bronchus at the hilum also underpins the classic “RALS” teaching rule (Right pulmonary artery Anterior, Left pulmonary artery Superior) used to orient students and trainees during bronchoscopy, mediastinal dissection, and interpretation of hilar masses or lymphadenopathy.
