Description
Running along the posterior vertebral column, the rotatores muscles appear as short, deep slips bridging from each transverse process to the spinous process of the vertebra immediately superior, most conspicuous in the thoracic region where they sit medial to the longissimus and beneath the spinalis. Superficial layers of the back, including trapezius and latissimus dorsi, frame the region, while the posterior deltoids and the posterior arm musculature extend laterally from the shoulder girdle. Individual vertebrae are delineated in the midline, and the posterior ribs arc laterally from the costovertebral joints, establishing the thoracic cage relationship to the spine. Depth matters here. Rotatores belong to the transversospinalis group (with multifidus and semispinalis) and are a frequent source of confusion in dissection because their fibers are short, segmental, and easily mistaken for multifidus at the thoracolumbar junction. Their oblique course from transverse to spinous process explains their contribution to segmental stabilization and contralateral rotation, a concept that becomes clinically concrete when teaching mechanical thoracic back pain or interpreting paraspinal changes adjacent to facet (zygapophysial) joint irritation. This posterior perspective also supports discussion of approaches that split the paraspinal muscles, such as posterior spinal exposure, where preserving midline attachments and recognizing the transversospinalis plane reduces unnecessary muscle trauma.
