Description
Proximal trachea descends in the midline and bifurcates at the carina into the right and left main bronchi, which diverge inferolaterally toward their respective hila. Cartilaginous rings and plates outline the bronchial wall, while the branching airway conduit continues as lobar and segmental bronchi in the expected asymmetric pattern, with a shorter, wider right main bronchus and a longer, more oblique left. Anterior and slightly inferior to the tracheal bifurcation, the heart occupies the central mediastinum, and the pulmonary trunk and pulmonary arteries course from the right ventricle to travel alongside the bronchi, with pulmonary veins returning oxygenated blood toward the left atrium. Relationship matters here, bronchi and pulmonary vessels run as paired bronchovascular bundles. That bronchovascular pairing is the point you teach and the landmark you operate on. Perihilar tumors, sarcoidosis, and infectious lymphadenitis enlarge hilar and subcarinal nodes and can narrow the main bronchi or distort the carina, a pattern often first appreciated on bronchoscopy or cross-sectional imaging. Intubation errors also live in this anatomy: the right main bronchus lies more vertical, so inadvertent right mainstem intubation preferentially ventilates the right lung and can collapse the left. Surgeons and interventional pulmonologists plan around the same map when selecting a bronchial segment for biopsy or when controlling pulmonary artery branches during lobectomy.
