Description
Seen from the inferior aspect in a male, the diaphragm spans the thoracoabdominal aperture with a central tendon positioned centrally and muscular crura descending posteriorly toward the upper lumbar vertebrae. Branches of the inferior phrenic arteries and accompanying veins course on the abdominal surface, running radially across the muscular periphery toward the central tendon and anastomosing across the left and right hemidiaphragms. Medially, vessels track near the caval opening and esophageal hiatus, with the phrenic vasculature approaching these apertures and their surrounding fibromuscular margins. Fine perforators cross the diaphragm toward the pleural side. A dense plexus. An inferior view matters when you are thinking like a surgeon approaching the diaphragm from the abdomen, where bleeding often comes from small-caliber but numerous phrenic branches rather than a single dominant trunk. This perspective supports teaching of why inferior phrenic arteries can become prominent feeders in hepatocellular carcinoma, and why transarterial embolization may target phrenic supply when a tumor abuts the bare area of the liver or invades the diaphragmatic surface. It also aligns with operative anatomy for hiatal hernia repair and esophagectomy, where dissection around the esophageal hiatus can encounter phrenic vessels and provoke troublesome oozing.
