Description
Arising from the spinal cord, paired dorsal (posterior) and ventral (anterior) nerve roots peel away segment by segment and converge to form mixed spinal nerves at each level of the vertebral column. The sequence tracks their bilateral emergence through the intervertebral foramina, bounded superiorly and inferiorly by adjacent pedicles and lined posteriorly by the zygapophysial (facet) joints, before the nerves divide into posterior and anterior rami along the paraspinal gutter. Dorsal root ganglia appear as focal enlargements on the posterior roots, positioned just proximal to the foraminal exit. Motion clarifies the repeating spatial pattern from superior to inferior levels. Compression happens here. Foraminal narrowing from posterolateral intervertebral disc herniation, uncovertebral osteophytes in the cervical spine, or hypertrophy of the facet joint can impinge the exiting nerve root and produce radicular pain in a dermatomal distribution, with motor deficits reflecting involvement of ventral root fibers. Animation makes the geometry easy to teach: as each root advances laterally, you can appreciate how little reserve space exists between the nerve, the annulus fibrosus, and the bony margins of the foramen, and why small changes in disc height can alter root caliber and angulation. A tight corridor. Use this sequence in neuroanatomy and musculoskeletal teaching blocks to connect spinal nerve anatomy to dermatomes and myotomes, or in radiology and spine surgery materials to orient learners to the foraminal “exit zone” seen on oblique MRI and CT. It also fits patient education for cervical or lumbar radiculopathy and preprocedural counseling for selective nerve root blocks or transforaminal epidural steroid injection.
