Description
Framed in lateral profile, the paired first ribs (costa I) sit at the superior margin of the thoracic cage, curving anteroinferiorly from the T1 vertebra toward their costal cartilages and the manubrium of the sternum. Just deep to the clavicular region, the short, broad shafts of the first ribs are positioned superior to rib II and anterior to the upper thoracic vertebral bodies, with the intervertebral discs separating adjacent vertebrae. Portions of the clavicle and scapula appear superior and posterolateral to the rib cage, clarifying the relationship between the shoulder girdle and the thoracic inlet. Costal cartilages form the anterior continuity of the ribs and stand out against the bony cortex. A lateral view of the first ribs matters because costa I defines the bony boundary of the thoracic outlet and carries key landmarks, including the scalene tubercle and the grooves for the subclavian vein (anterior) and subclavian artery with the inferior trunk of the brachial plexus (posterior). Small shifts in this anatomy underlie thoracic outlet syndrome, where neurovascular compression can occur between the anterior and middle scalene muscles or against an anomalous cervical rib. The first rib is also a surgical and trauma landmark. It is uncommon to fracture in isolation, so a first rib fracture raises concern for high-energy injury and associated vascular damage.
