Description
Arcing from the upper thoracic vertebrae, the first ribs (costae I) are shown in lateral profile as short, broad, strongly curved bones forming the superior margin of the thoracic cage. Each rib articulates posteriorly with the body of T1 at the costovertebral joint and with the transverse process at the costotransverse joint, then sweeps anteriorly toward its costal cartilage, which is distinguished from bone by a contrasting tint. Along the superior surface, the scalene tubercle divides the grooves for the subclavian vein (anterior) and subclavian artery (posterior), while the head, neck, and tubercle of the rib sit immediately lateral to the vertebral column. Clinically, the first rib’s morphology matters because it sets the boundaries of the thoracic inlet and provides fixed landmarks for the subclavian vessels and lower trunk of the brachial plexus as they traverse the costoclavicular space. Hypertrophy or anomalous attachment of scalenus anterior, or elevation of the first rib after trauma, can narrow this corridor and contribute to neurogenic or vascular thoracic outlet syndrome; the grooves and scalene tubercle are the bony correlates of that compression pathway. Surgeons also use the first rib and its costotransverse region as reference points during supraclavicular decompression and first rib resection, where orientation to the T1 articulation helps avoid pleural and neurovascular injury. Useful for gross anatomy and osteology teaching when introducing costovertebral joint mechanics and the distinctive features that separate costa I from typical ribs (II to IX). It also fits radiology and surgery materials that explain apical chest trauma, thoracic inlet anatomy, and the bony substrate of thoracic outlet syndrome, including correlation with CT or plain film identification of the first rib contour and cartilage.
