Seen in lateral profile, ribs XI and XII (costae fluctuantes) arc posteriorly from their costovertebral and costotransverse joints on the lower thoracic vertebrae, then run anteroinferiorly as relatively straight bony shafts. Their anterior (distal) ends terminate free in the posterolateral abdominal wall, unlike ribs I to X, and they sit inferior to rib X with a clear step-down in length and curvature. Posterior elements of the thoracic spine are visible in series, with intervertebral discs separating adjacent vertebral bodies and setting the segmental level for each rib head and tubercle. Because the floating ribs lack anterior attachment to costal cartilage and sternum, they are more mobile and more often implicated in focal chest wall pain from minor trauma, muscle strain, or slipped rib and costovertebral dysfunction. This lateral perspective clarifies how the distal tips project toward the flank and upper abdomen, a practical point when localizing fractures on plain radiographs or CT, or when planning a posterolateral approach where the 11th and 12th ribs serve as surface landmarks. Their proximity to the diaphragm and upper retroperitoneum also explains why lower rib injury can coexist with renal or splenic trauma, depending on side.
