Description
Curving tightly around the superior thoracic aperture, the first ribs (costae primae) sit superior to the second ribs and slope inferoanteriorly from their vertebral attachments toward the manubrium. Posteriorly, each rib articulates with the body of T1 and its transverse process at the costotransverse joint, while the rib head and neck blend laterally into a flattened shaft. From a lateral perspective the rib’s superior surface is appreciable, including the scalene tubercle, with the subclavian vein groove lying anterior to it and the subclavian artery groove posterior to it. The thoracic vertebrae stack immediately medial to the rib heads, with intervertebral discs separating adjacent vertebral bodies and the thoracic kyphosis carrying the cage anteriorly. This is the rib that sets the tone for thoracic inlet anatomy. Its short length, broad contour, and proximity to the subclavian vessels and brachial plexus explain why a cervical rib or fibrous band at this level can contribute to thoracic outlet syndrome, and why first rib resection is performed through transaxillary or supraclavicular approaches in refractory cases. Fracture patterns also differ from lower ribs; a first rib fracture implies high-energy trauma and raises concern for adjacent vascular injury. Small rib, big implications. Use this lateral view when teaching surface and sectional anatomy of the superior mediastinum and thoracic wall, or when diagramming the thoracic inlet for trauma, vascular, and peripheral nerve surgery texts. It also suits radiology education that pairs skeletal landmarks with apical lung and subclavian vessel relationships on CT and plain films.
