Description
Seen from the medial aspect of the male foot and distal leg, the medial marginal vein begins along the medial border of the great toe and runs proximally along the medial margin of the foot toward the arch. It tracks anterior to the medial calcaneal region and then ascends in the subcutaneous tissues anterior to the medial malleolus, where it continues as the great saphenous vein. Tributary venules from the dorsal venous network and plantar venous plexus converge on this channel, outlining the superficial venous drainage pathway along the medial column. Clinically, this segment matters because the vein’s course over the first metatarsal and past the medial malleolus is a common starting point for mapping the great saphenous vein before endovenous ablation or surgical stripping for chronic venous insufficiency. The medial malleolus region is also where the great saphenous vein lies close to the saphenous nerve, so vein cannulation, cutdown, or varicosity phlebectomy here can produce a characteristic medial leg dysesthesia. Landmark clarity. Superficial thrombophlebitis and varicosities often declare themselves along this medial pathway, making a medial perspective a practical teaching angle.
