Description
Along the medial aspect of the male foot, the medial plantar vein runs deep in the plantar compartment, accompanying the medial plantar artery and paralleling the course of the medial plantar nerve after its division from the tibial nerve in the tarsal tunnel. Proximally, the vessel track relates to the flexor retinaculum and neurovascular bundle posterior to the medial malleolus, then continues distally along the medial sole toward the hallux-side of the forefoot. More superficial medial venous channels of the foot and lower leg may be included for orientation, with the plantar system shown in a deeper plane relative to the plantar aponeurosis and intrinsic musculature. Small tributaries converge into the medial plantar venous pathway. A tight corridor. For teaching and procedural planning, a medial perspective clarifies the relationship between plantar venous drainage and the structures clinicians palpate and decompress in the retromalleolar region, where posterior tibial artery pulse, tibial nerve symptoms, and varicosities can coexist. This anatomy matters in tarsal tunnel syndrome workups and in surgical exposure for posterior tibial artery bypass, tibial nerve release, or repair of lacerations along the medial sole, where inadvertent injury to the medial plantar vessels can complicate hemostasis and wound healing. It also supports interpretation of duplex ultrasound mapping when tracing plantar venous outflow toward the posterior tibial veins.
