Description
Oriented palm-down in a posterior (dorsal) view, the distal forearm transitions into the dorsum of the hand with the extensor digitorum muscle giving rise to four tendons that course distally toward digits II to V. Each tendon is invested by a synovial tendon sheath (vagina synovialis tendinis) and lies superficial to the carpal bones and dorsal intercarpal ligaments as it approaches the metacarpophalangeal joints. Proximally at the wrist, the tendons pass deep to the extensor retinaculum, then fan out over the metacarpals, with the index-side tendons positioned more radial and the little-finger tendon more ulnar. Skin removal exposes the layered relationships clearly. Posterior tendon sheaths matter most where friction is highest, under the extensor retinaculum and across the dorsal wrist, where tenosynovitis can present with pain, crepitus, or triggering during finger extension. A classic correlate is fourth dorsal compartment extensor tenosynovitis, sometimes seen in overuse athletes and in inflammatory arthropathies such as rheumatoid arthritis, where synovial proliferation can compromise tendon glide and predispose to attritional rupture. This view also supports teaching the compartmental logic used in dorsal wrist ultrasound and in surgical approaches for dorsal ganglion excision or tendon synovectomy.
