Description
Viewed from a superior angle after calvarial cutaway, the basilar venous plexus occupies the midline on the clivus, deep to the dura mater and just anterior to the foramen magnum. It lies inferior to the dorsum sellae and posterior to the body of the sphenoid, forming a venous network that communicates laterally with the inferior petrosal sinuses along the petroclival fissure. Posteriorly, venous channels converge toward the region of the confluence and transverse sinuses, while smaller venules trace the grooves and dural attachments across the cranial fossae. Bone landmarks of the anterior, middle, and posterior cranial fossae frame the plexus and its outflow routes. Skull base venous anatomy matters because the basilar plexus is a recognized conduit between intracranial dural sinuses and the internal vertebral venous plexus, creating a potential pathway for infection or tumor spread from head and neck to the posterior fossa and cervical spine. In endoscopic endonasal and transclival surgery, this plexus can be a source of persistent, low pressure bleeding that obscures the operative corridor and complicates dural closure. Its relationship to the inferior petrosal sinus also intersects real procedures, including inferior petrosal sinus sampling and transvenous access routes, where variant channels or prominent intercavernous and basilar connections can change catheter behavior. A surgical nuisance. Use this superior perspective for teaching dural venous sinus drainage in neuroanatomy and head and neck modules, or for illustrating petroclival and foramen magnum approaches in neurosurgery and neuroradiology texts. It also suits CME graphics on venous pathways implicated in cavernous sinus thrombosis, posterior fossa dural arteriovenous fistulas, and skull base infection spread patterns.
