Seen from a superior cranial base perspective, the ethmoidal vein is traced along the ethmoid labyrinth at the medial orbital apex, coursing posteriorly toward the anterior cranial fossa. Medial to the orbit, the cribriform plate and crista galli sit centrally, with the ethmoidal foramina along the frontoethmoidal suture marking the passageway for the anterior and posterior ethmoidal vessels and the nasociliary nerve branches. Posteriorly, the dura based venous channels of the skull base are rendered in blue, allowing the ethmoidal venous outflow to be understood in relation to the cavernous sinus region and adjacent dural sinuses rather than as an isolated orbital vessel. Orientation is clear. The foramen magnum anchors the inferior midline. This superior approach matters because the ethmoidal veins participate in a clinically relevant communication between the nasal cavity, paranasal sinuses, orbit, and intracranial venous sinuses, a pathway implicated in the spread of infection from ethmoid sinusitis into the orbit or cavernous sinus (including septic cavernous sinus thrombosis). For surgeons working endoscopic sinus procedures or orbital decompression, the small caliber of the ethmoidal vessels belies their importance, uncontrolled bleeding at the ethmoidal foramina can obscure the skull base and increase risk at the cribriform plate. The venous anatomy also frames why fractures across the anterior cranial fossa may present with orbital ecchymosis and CSF leak in the same clinical encounter.
