Description

Running along the medial orbital wall, the anterior and posterior ethmoidal veins traverse the ethmoidal foramina near the frontoethmoidal suture, linking the ethmoid air cells to the orbital venous drainage. Medially, these vessels relate to the cribriform plate of the ethmoid and the floor of the anterior cranial fossa, while laterally they communicate with the superior ophthalmic vein within the orbit. Superiorly and posteriorly, their outflow is shown in continuity with the dural venous sinuses within the dura mater, with the superior sagittal sinus included as a midline landmark along the attached margin of the falx cerebri. Small veins. Big consequences. Because the ethmoidal veins connect the sinonasal region with the orbit and intracranial venous system, they sit on a clinically sensitive crossroads for infection spread and bleeding control. Ethmoiditis can progress to orbital cellulitis or, via valveless venous pathways, to intracranial complications such as meningitis or cavernous sinus thrombosis through the ophthalmic venous network. This relationship also matters in endoscopic endonasal surgery and anterior skull base approaches, where the ethmoidal arteries and companion veins near the anterior ethmoidal canal guide safe dissection and help limit hemorrhage. Use this artwork to teach venous drainage of the anterior cranial fossa in neuroanatomy, head and neck anatomy, or ophthalmology blocks, and to clarify orbit to sinus to dura connections in radiology correlation sessions (CT of the ethmoid labyrinth, MR venography of dural sinuses).
