Description
Seen from a superior perspective, the L2 vertebra presents a broad vertebral body anteriorly, with the vertebral arch sweeping posteriorly to enclose the vertebral foramen. Medial to the transverse processes, the pedicles and laminae define the lateral and posterior margins of the spinal canal, while the spinous process projects posteriorly in the midline. Dominating this view are the paired superior articular processes, set posterolaterally to the vertebral foramen, their superior articular facets oriented mainly posteromedially toward the adjacent L1 inferior facets. Facet orientation is the point. In the upper lumbar spine, the zygapophysial (facet) joints guide flexion and extension while limiting axial rotation, and subtle differences in facet angulation are discussed in the setting of degenerative facet arthropathy and segmental instability. This superior view also matches the bony landmarks referenced during posterior approaches, when surgeons or interventionalists work around the lamina, pars interarticularis region, and facet joint to perform decompression, place pedicle screws, or target the medial branch nerves that innervate the L1 to L2 and L2 to L3 facet joints.
