Description
Seen from a superior perspective, an intrasegmental bronchus is centered as it continues distally from a segmental (tertiary) bronchus and divides into smaller conducting airways consistent with subsegmental bronchi. Short, cartilaginous-walled branches radiate outward, their lumina oriented inferolaterally as they course toward the peripheral lung parenchyma. Proximally, the airway axis implies continuity back toward the lobar bronchus and mainstem bronchus of the respiratory tract, while the more distal branches narrow as smooth muscle becomes the dominant wall component. Branching geometry is the message. Teaching segmental anatomy often fails when learners cannot connect names to three-dimensional airway orientation, and a top-down view resolves that problem by matching what you face during bronchoscopy when the scope is advanced and rotated to enter a given segment. This perspective also helps explain why aspirated material and endobronchial obstruction distribute along predictable pathways within a bronchopulmonary segment, and why postobstructive atelectasis can remain sharply confined until collateral ventilation intervenes. For thoracic surgery, the relationship between subsegmental bronchi and the planned intersegmental plane underlies anatomic segmentectomy and guides stapler placement. Clean margins depend on this.
