Description
Arising from the left mainstem bronchus, the left upper and lower lobar bronchi give rise to tertiary (segmental) bronchi whose intrasegmental branches ramify within the bronchopulmonary segments. From a left lateral view, the bronchial tube walls and sequential bifurcations are emphasized, with distal branches tracking laterally and inferiorly as they taper toward bronchioles. Segmental bronchi remain more central and medial, while the intrasegmental bronchus courses within the segment, accompanied in life by segmental pulmonary arteries running alongside it. Segmental anatomy is the practical unit for pulmonary resection. Surgeons performing a left segmentectomy (for example, resection of the superior segment of the lower lobe, S6, or a lingular segment) follow the segmental bronchus as a reliable landmark, then divide its intrasegmental branches while preserving adjacent segments. Misidentifying a tertiary branching pattern can leave a devascularized or atelectatic segment, and endobronchial procedures can also hinge on this map when selecting the correct segment for bronchoscopy, lavage, or biopsy. Side-on orientation clarifies how quickly the left bronchial tree transitions from lobar to tertiary branching compared with the longer right-sided mainstem configuration.
